Registered Nursing Services

Evergreen’s registered nurse is available to provide in-home visits at your request for care coordination and consultation.

Registered nursing services may include the following:

• Nursing assessments, evaluations, care plans and client care conferences
• Supervision and oversight of staff providing personal care & medication administration services, including delegated nursing tasks
• Care management and coordination with physicians, home health, hospice, pharmacists, family members and other community partners
• Addressing specific changes in client care needs, conditions and special treatments
